    1. Work directly under the supervision of the Medical Records Supervisor.

    2. Encodes letters and reports, birth and death certificates.

    3. Encodes Emergency Room (ER) Records in the Hospital Registry.

    4. Encodes Admitted and Discharged patients in the Hospital Registry.

    5. Arranges and assembles the medical records of discharged patients of the hospital.

    6. Collects and compiles data from medical records for statistical and other related reports.

    7. Review medical records to ensure that required reports and signature have been included. (Qualitative and Quantitative analysis).

    8. Maintains surveillance of incomplete medical records.

    9. Process birth and death certificates

    10. Files records according to established and approved systems.

    11. Incorporate loose sheets/reports to respective charts.

    12. Maintain a follow- up system for borrowed and needed chart.

    13. Retrieve requested records for follow-up, research and studies.

    14. Maintain the proper filing of the medical records.

    15. Answer telephone inquiries.

    16. Reproduction of documents as request on window

    17. Assists patients/relatives for the request of medical certificate, clinical abstract and other documents.

    18. Photocopy/ Stamping of charts for PHIC Claims.

    19. Scanning of files for electronic medical records filing.

    20. Interview patients and reporting online on Unified Disease Registry System(stroke, copd, cancer, diabetes, blindness, cad)

    21. Interview patients and reporting online on Online National Electronic Injury Surveillance System (trauma, injury).

    22. Monitoring and maintenance of Data Management Record for Electronic Medical Record Filing System.

    23. Perform other related functions as may be assigned by the immediate supervisor.
