  • FUNCTIONS, DUTIES AND RESPONSIBILITIES
    1. Attends court proceedings and represent the hospital in court cases involving subpoena of medical / clinical records.
    2. Represents the department to top management.
    3. Exercise direct administrative supervision and control over all subordinates in the department.
    4. Establishes policies and procedures for the content, control, storage and retrieval of records.
    5. Ensure the maintenance of the patient’s right to privacy and confidentiality.
    6. Organizes workflow throughout the service.
    7. Meets and discusses with other departments all issues related to medical records section.
    8. Serves on appropriate committees and attend meetings of relevance to the medical record section.
    9. Answers by correspondence or by telephone inquiries regarding information recorded on the patient’s charts.
    10. Plan staff, space, and equipment of the department.
    11. Assist the medical staff in research studies.
    12. Keep abreast of current medical records practices.
    13. Analyze specific portions of the medical record and assign code numbers to diseases based on the accepted classification system.
    14. Review medical records to ensure that required reports and signatures have been included (Quantitative and qualitative analysis).
    15. Maintain Surveillance of incomplete medical records and prepare reports of delinquent doctors.
    16. File and indexes according to the established and approved system.
    17. Maintain the proper filing of the medical records.
    18. Answers telephone inquiries.
    19. Performs accurate coding of in-patient and out-patient medical records.
    20. Final Checking of charts for PHIC Claims.
    21. Encodes PHIC members in registry system.
    22. Monitors and maintains Data Management Record for Electronic Medical Record Filing System.
    23. Collate part C TO Part H of the Mandatory Monthly Hospital Report submitted every10th of the month to Philippine Health Insurance.
    24. Collate Monthly Statistical Reports (DOH) reports from the Laboratory, X-ray, ER, OPD, and other ancillary department and submits to the president Office.
    25. Online reporting of the Unified Disease Registry System.
    • Integrated Chronic Non-Communicable Disease Registry System ( Blindness, Diabetes, Chronic Obstructive Pulmonary Disease, Coronary Artery Disease, Cancer, Stroke)
    • Online National Electronic Injury Surveillance System
    26. Online reporting of Department Health Hospital Statistic report ( Health Facilities and Services Regulatory Bureau)
    27. Be courteous, helpful and polite to all patients at all times.
    28. Attend and participate in meeting as called for by the administrator.
    29. Prepare and submit report, memoranda to the Administrator and other departments of the hospital as requested.
    30. Establish and maintain effective working relationships with co-workers, supervisors and the general public.
    31. Perform duties in an efficient, professional and courteous manner.
    32. Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health & safety procedures.
