JOB SUMMARY : Responsible for monitoring, return and disposal of expiring and expired
medicines. Makes monthly summary of PWD and Senior Citizen’s discount, supervises Pharmacy Assistants in dispensing drugs, and ensures the availability of drugs for in-patients and replaces e-carts.

DUTIES AND RESPONSIBILITIES:
1. Daily monitoring of refrigerator and room temperature.
2. Double-checks the deliveries of medicines at General Stockroom together with the Audit staff and the GSR staff.
3. Prepares request of medicines and IV fluids from General Stockroom.
4. Checks and receives medicines and IV fluids delivered by General Stockroom staff.
5. Prepares list of drugs for emergency purchase which are not carried and/or out-of stock in the pharmacy.
6. Charges and prepares medicines of outpatients with or without HMO.
7. Prepares and labels medicines of in-patients.
8. Validates all doctors’ prescription received in pharmacy.
9. Double-checks all medicines to be dispensed prepared by the Pharmacy Assistants.
10. Updates the Medication Monitoring Sheet of newly admitted patients and all in-patients under Unit Dose Drug Distribution System.
11. Dispenses medicines to in-patients under Unit Dose Drug Distribution System good for 24 hrs.
12. Prepares the medication with corresponding label of in-patients under Unit Dose Drug Distribution System.
13. Double checks the prepared medicines under Unit Dose Drug Distribution System before its delivery to all nursing stations.
14. Endorses the prepared medicines under UDDDS to the nurse-on-duty.
15. Post-charges all medicines delivered to all nursing stations under UDDDS.
16. Responsible for storage and stability of all medicines in the pharmacy.
17. Monitors the quantity of all released vials and returned empty vials in every shift.
18. Prepares medicines and IV fluids as replacement of those used at the OR and NICU/PICU and those used in post charges of ER, ICU, all nurse stations, and MRI.
19. Prepares regulated and dangerous medicines as replacement on e-cart of those used at the OR, NICU-PICU, ICU, ER and all nursing stations and MRI.
20. Weekly monitoring of assigned medicines and reports the critical and out-of-stock of such to the Chief Pharmacist.
21. Monitors the expiration date and facilitates monthly inventory of assigned medicines.
22. Facilitates credit memo of returned medicines of in and out-patients.
23. Summarizes filled and unfilled prescriptions counting per shift.
24. Monitors and requests office supplies from General Stock Room.
25. Prepares and returns expired medicines to General Stockroom.
26. Responsible in pulling-out of loose expired and damaged or broken medicines from the system through quantity adjustment.
27. Monitors and prepares report of expiring and expired medicines and coordinate the return or replacement of such to drug companies concerned.
28. Responsible for the proper disposal of loose expired medicines that cannot be returned to supplier, as well as the damaged or broken medicines.
29. Files Credit Memo of expired medicines.
30. Secures old files and documents of pharmacy and forwards to Housekeeping Department for keeping and storage.
31. Makes monthly summary of PWD and Senior Citizen’s discount to be forwarded to Finance and
Purchasing Department.
32. Undertakes 5S in the workplace.

• Minimum education level: Bachelor´s Degree
• 1 year of experience
