    1. All treatment should be done within 8:00am – 12:00nn and 1:00pm – 5:00pm from Mondays – Saturdays. Cut – off time for treatment is 8:00am – 4:00pm.

    2. A patient will not be treated without the patient chart containing their referral/documents, to ensure safety of the patient and protection of the therapist.

    3. The PT/OT staff should have mastery of the patient’s chart at all times and review the chart prior to treatment and/or evaluation to avoid patient mismanagement. Any questions regarding the patients are directed to the Physiatrist immediately. When in doubt, always consult a physician.

    4. All patient should be treated with the due respect and courtesy. A professional attitude must be exhibited at all times. Rapport with the patient should not be confused with over familiarity. Patients, caregivers or anyone who harass the staff should be reported immediately to the Security and inform the Physiatrist.

    5. The PT/OT staff should ensure that all pieces of equipment to be used are working before treatment is started. Any equipment that is not working schedule be reported to Physiatrist immediately. The service technician may contacted right away.

    6. Adequate supervision should be given to the patient during treatment, and proper precautions should always be observed to ensure patient safety. Any untoward incidents (falls, burns, fractures, unmonitored vital signs leading to erratic and extreme blood pressure changes/autonomic dysreflexia and any other injury brought about by any procedure) must be reported immediately to the Physiatrist. The PT/OT staff, narrating the events that took place and signed accordingly, shall make an incident report.

    7. Inquiries made by the patient/and or members of the patient’s family regarding the medical condition should be referred to the Physiatrist. Information about the patient is strictly confidential and should be discussed only within the professional involved in the treatment of the patient. The patient is not allowed to read his/her own chart.

    8. Proper aftercare should be observed after treatment of every patient. All equipment used should be returned to their proper places. Failure to perform aftercare procedures will be dealt with accordingly.

    9. All equipment/materials should not be taken out of the Department unless for use by the PT/OT staff for bedside treatment.

    10. Attends and participates in meetings and seminars.
