  • DUTIES AND RESPONSIBILITIES:
    • Receives complete endorsement from the outgoing shift and endorses patient to the incoming shift.
    • Accepts patient for admission from the emergency room and/or other unit. Verifies from the endorsing unit for the need of mechanical ventilator set up if necessary and verifies if the CPD was informed.
    • Helps to transfer the patient to bed safely considering their right of privacy and keeps them comfortable. Takes initial assessment and V/S. Assists in hooking patient to mechanical ventilator, cardiac monitor and other medical equipment if needed.
    • Prepares patient’s chart and flow chart. Fills up all necessary forms for documentation and all identification card for patient directory.
    • Orients patient and relatives or significant others of the hospital policies and unit protocol.
    • Carries out doctor’s order, transcribes to Kardex, writes all medications in the medication sheet, IVF / BT in the IVF sheet etc.
    • Performs thorough and systematic assessment, intensive monitoring of vital signs an neuro – vital signs, fluid intake and output, identifies verbal and non-verbal cues and watches out for sudden or subtle changes in the patient’s condition that might require emergency intervention - refers immediately to ROD/AP.
    • Assesses skin integrity of bed ridden patient to prevent skin ulcers, risk of complications due to nutritional status, pseudomonas, etc. Does morning care and keeps the patient dry at all times.
    • Writes nurse’s notes and documents all nursing activities and observations to the patient’s condition that has transpired during the shift following nursing process (SOAPIE).
    • Sensitive to emotional and spiritual needs of the patient. May give status updates (such as latest vital and neuro – vital signs, drips) to the immediate family members so that they can make decisions regarding treatment but not to divulge sensitive issues pursuant to ethical and hospital policy.
    • Administers IV Therapy treatments like medications, IVF, TPN, BT, etc. as prescribed by the doctor following the 12 rights of giving medications. Be alert of untoward signs and symptoms like allergy and refers immediately to ROD/AP.
    • Observes universal precaution for self-protection and practices medical asepsis and proper waste segregation following infection control policies/guidelines.
    • Accompanies ROD/AP during their routine rounds and assists them on their treatment or bedside procedure.
    • Manages/monitors patients’ mechanical ventilator support, cardiac monitor and other types of medical equipment for its proper functioning.
    • Prepares the patient for various diagnostic test like Ultra Sound, 2-D Echo, CT scan, MRI, blood chemistries, etc. and analyzes/relies result to ROD/AP for emergency intervention if needed.
    • Collaborates with fellow members of the critical care team and maintains harmonious relationship among the staff and other hospital personnel.
    • Responds to life-saving situations (call for CODE) using nursing standards and protocols for treatment whenever help is needed in other units/ward.
    • Helps in training and supervising nurse trainees and new staff nurses in the performance of their duty.
    • Keeps the unit or working area clean and in order at all times to have a safe and contamination free environment to prevent possibility of nosocomial infection.
    • Performs post-mortem care and informs the nursing aide to transfer the cadaver in the morgue.
    • Acts as advocate to patient and coordinates for medical conferences related to patient care.
    • Prepares patient for surgical procedure following the pre-operative checklist and hospital policy/protocol.
    • Performs nursing procedures and techniques with confidence. Asks the senior nurse if uncertain.
    • Facilitates the discharge of patient as ordered and prepares home instructions and health teachings.
    • Does other task that are assigned by the head nurse like inventory of supplies in E- cart, IV fluids, CSR supplies, forms, keeping of patient’s record, preventive maintenance of equipment and facilities. Requests a job order if it needs repair.
    • Wears proper uniform and identification card at all times while inside the hospital vicinity.
    • Responsible to self development and educational advancement. Participates in investigations related to improving nursing care.
    • Attends monthly unit meeting, quarterly general assembly, in-service lectures and institutional activities if necessary.
    • Adheres to institutional policies and procedures, nursing standards and regulations and keeps the head nurse/supervisor informed of needs/problems that may encountered.
